The Sentara Health Plans Vantage HMO Plan is a plan choice in the Greater Hampton Roads area available to Commonwealth of Virginia and The Local Choice employees where an employer has selected Sentara Health Plans as a plan offering.  It’s an affordable, low cost plan that includes all of your benefits - medical, pharmacy, expanded dental, vision, and hearing. With the Sentara Health Plans Vantage Plan, members may receive services at participating providers in the Sentara Health Plans Vantage network throughout Virginia and North Carolina.

Employees enrolling with Sentara Health Plans must live or work in the Greater Hampton Roads region or on the Eastern Shore, an area defined as: Accomack, Chesapeake, City of Franklin, Gloucester, Hampton, Isle of Wright, James City, Mathews, Newport News, Norfolk, Northampton, Poquoson, Portsmouth, Southampton, Suffolk, Surry, Virginia Beach, Williamsburg, York. Enrollment opportunities are available at the annual election period for your employer group or during a special enrollment period following a qualifying life event.
