Our diabetes and heart C-SNP supports members living with diabetes, chronic heart failure, or cardiovascular disease. Eligible members may receive diabetic supplies, like $0 insulin, or cardiac and pulmonary rehabilitation services.
Sentara Medicare Engage – Lung
$0 monthly premium
This C-SNP is only available in Hampton Roads and supports members living with asthma, chronic bronchitis, emphysema, pulmonary fibrosis, or pulmonary hypertension.
