Non-Participating Providers must register and update information with Sentara Health Plans using the Non-Participating Provider Registration Form. We strongly advise new Non-Participating Providers to enroll prior to submitting claims. Claims received without receipt of the Non-Participating Provider Registration form with attached W-9 first will be rejected. Once confirmation has been received that the enrollment form has been processed, claims can be resubmitted.
Please Note: Providers contracted with Sentara Health Plans should navigate to the Update Your Information page to update information. Contracted providers that submit using the Non-Participating Provider Registration Form will not be processed and returned.
Important note: Receiving confirmation of enrollment after claims have been submitted will still result in claims being rejected.
