To serve you and our members effectively, it's important that we have up to date information about your practice. Please notify Sentara Health Plans as soon as possible of any changes related to your practice’s operations by submitting the appropriate update form.
Note: Any requests to credential or add new (ARTS and MHS - legacy CMHRS) services for existing providers/groups (contracted TIN's), via this Provider Update Form will be rejected. These requests must be submitted to Contract Management/your Contract Manager for review and approval. Requests to join the network should be submitted via the Request for Participation form.
