Payment policy #5006 is a new policy for the Commercial line of business.

As part of our ongoing effort to provide clear and transparent billing and coding guidance, payment policies that apply to multiple lines of business may be separated into distinct policies. As a result, providers may see updated policy numbers and/or naming conventions. Providers should refer to the Policy History section of each payment policy or the Payment Policy portal for additional information.
